Club Memberships

Clubs must register each new membership year which starts September 1st.

Please fill out the Club Membership Registration Form and mail with the Annual Club Registration Fee, payable to Field Hockey Ontario, to the address at the bottom of the form.

Once the club registration form and fee have been received by FHO, the two listed club contacts (who must be current FHO members themselves) can request access to their club's roster for the current membership year.
